If a physician is inactive, former, etc., membership and privileges must be requested in writing and addressed to the Medical Staff Services Office, 1225 North State Street, Jackson, MS  39202.  

Unless otherwise stipulated in writing by the physician, the release of information signed by the physician at appointment or reappointment covers the verification of this information. (approved MEC 1/11/00)
